Health Department offers COVID-19 Vaccination information form for priority groups

An inordinate amount of traffic temporarily overwhelmed the Benzie-Leelanau District Health Department’s website this weekend (it’s back online now), which prompted the department to post a direct link to their COVID-19 Vaccination Information Form.

The health department reports that it is currently working to vaccinate priority group 1A (paid and unpaid persons serving in healthcare settings who have the potential for direct or indirect exposure to patients or infectious materials and are unable to work from home as well as residents in long term care facilities). If you are in priority 1A, please complete the form so that you can be scheduled with the health department’s current vaccine allocation.

If you are an employee or employer in priority group 1B, you may still fill out the form as the health department gathers information to plan for the next phase. The department will not (yet) reach out to you for scheduling vaccine until it is ready for that priority group. Please visit Bldhd.org for updates.
